Job Summary
Provide medical nutrition therapy for patients with type 2 diabetes and prediabetes, and deliver individualized DSMES sessions. Educate patients on CGM devices, interpret glucose data, and collaborate with providers to optimize therapy. Develop culturally responsive meal plans, support patients navigating food insecurity, and collaborate with an interdisciplinary care team to advance a Chronic Disease Management Program. Opportunity to influence program design, curricula, patient education materials, and outcome tracking, with CDCES exam preparation and conference attendance supported as part of professional development.
Required Qualifications
