Job Summary
Provide Diabetes Self-Management Education and Support, including Medical Nutrition Therapy for patients with diabetes and other nutrition-related conditions. Conduct comprehensive nutrition assessments and develop individualized care plans using the Nutrition Care Process. Collaborate with interdisciplinary teams to optimize outcomes, coordinate referrals for food access programs, and support quality improvement initiatives. Deliver culturally responsive education and monitor patient progress within the electronic health record. This full-time role involves Monday through Friday hours from 8:00 a.m. to 5:00 p.m. with occasional flexibility for client scheduling demands. The position requires a Registered Dietitian Nutritionist credential and a preferred CDCES certification.
Required Qualifications
- Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration (CDR)
- Current licensure as a Dietitian in the State of Iowa
- Current BLS or CPR/First Aid certification
- Current mandatory reporter training for child and dependent adult abuse certificate
- Experience providing Medical Nutrition Therapy in an outpatient or primary care setting
- Experience in a Federally Qualified Health Center (FQHC), community health center, or other underserved setting
- Experience with diabetes technology, including continuous glucose monitoring (CGM) and insulin pump education
- Experience with HRSA quality measures, population health initiatives, and addressing social drivers of health
- Working knowledge of MS Office Products (ex. Excel, Word, Power Point) and Windows Platform
- Ability to work independently within established protocols, provider recommendations, and RDN scope of practice
- Ability to respond calmly in stressful situations
- Ability to work with diverse groups of people
- Strong analytical, problem-solving, and reasoning skills
- Strong communication skills, including listening, speaking, writing, and reading comprehension
- Self-motivated to develop and maintain own competence as it pertains to the position
- Maintains strict confidentiality
- Skilled in planning, organizing, adaptability, attentiveness to detail and flexibility of assignment
- Occasional driving might be expected for this position
Desired Qualifications
- Certified Diabetes Care and Education Specialist (CDCES) credential preferred at the time of hire but required to obtain within twenty-four (24) months of hire, with employer support for certification and continuing education
- Experience providing Medical Nutrition Therapy in an outpatient or primary care setting preferred
- Experience in a Federally Qualified Health Center (FQHC), community health center, or other underserved setting preferred
- Experience with diabetes technology, including continuous glucose monitoring (CGM) and insulin pump education preferred
