Job Summary
Conduct clinical nutrition assessments, document findings in the EMR using the Nutrition Care Process, and complete MDS - Section K. Collaborate with the interdisciplinary care plan team to make recommendations for wounds and significant weight changes, including calculating Total Fluid requirements. This role supports our long-term care community in San Leandro, CA, with a flexible 16-hour weekly schedule and excellent pay.
