Job Summary
Conduct clinical nutrition assessments, document using the Nutrition Care Process in the EMR, and complete MDS Section K for our long-term care community in Lebanon, NH. Collaborate with the interdisciplinary care plan team to make recommendations regarding wounds and significant weight changes, including calculating total fluid requirements. This role requires CDR credentialing and dietetic licensure, with a flexible schedule of 16-18 hours weekly.
