Job Summary
Conduct clinical nutrition assessments, document using the Nutrition Care Process in the EMR, and complete MDS - Section K. Collaborate with the interdisciplinary care plan team, calculate TF, and make recommendations for wounds and significant weight changes. Requires CDR credentialing, dietetic licensure, and client-centered communication skills. Schedule is 16-24 hours per week with flexible hours.
