Job Summary
Assess patient nutritional status and develop individualized nutrition care plans based on medical records, lab values, and interdisciplinary input. Implement clinical interventions per provider orders while monitoring patient response and adjusting plans as needed. Provide nutrition education and counseling to patients, caregivers, and staff, and document all interactions in the electronic health record. Collaborate with the interdisciplinary team on Quality Assessment and Performance Improvement activities and contribute to unit meetings and outreach initiatives. Coordinate care with nurses, physicians, and external providers to support patient outcomes.
Required Qualifications
- Bachelor's degree
- Must be a Registered Dietitian credentialed by the Commission on Dietetic Registration (CDR)
- Must be certified or licensed and registered in the state where services are provided, in accordance with applicable state requirements
- Must have a valid driver's license
- Must be available for Monday - Friday shifts
- Must be available for Part-time (0.2FTE) schedule
- Must be able to work on-site in Portland, OR
- Must be able to work in a safe work environment subject to pre-employment background checks, drug screenings (non-THC), employment verification and references
- Must be able to receive the COVID-19 vaccine
Desired Qualifications
- Approximately one (1) year of professional experience in clinical nutrition or a related setting
