Job Summary
Provide nutritional assessments and ongoing evaluation of the plan of care for LTC/SNF residents; participate in IDCP care conferences; provide nutrition education and counseling to residents, families, and staff; collaborate with interdisciplinary teams to improve residents' health; ensure compliance with regulations; requires RD or RD eligible; LTC/EMR experience preferred.
