Job Summary
Provide nutritional assessments and ongoing evaluation of the plan of care for residents in LTC settings; participate in IDCP care conferences as needed; provide nutrition education and counseling to residents, families, and staff; collaborate with interdisciplinary teams to improve residents' overall health and well-being; demonstrate ability to work cooperatively as a team member; complete other duties as assigned; ensure compliance with federal, state, and local regulations and guidelines.
Required Qualifications
- Registered Dietitian or RD eligible
- organizational skills
- excellent communication and interpersonal skills
- experience in LTC and EMR preferred
