Job Summary
Provide nutritional assessments and ongoing evaluation of the plan of care for residents in LTC/SNF/subacute facilities. Participate in IDCP care conferences as needed. Deliver nutrition education and counseling to residents, families, and staff. Collaborate with interdisciplinary teams to improve residents' health and well-being. Demonstrate teamwork and complete other duties as assigned while ensuring compliance with federal, state, and local regulations.
Required Qualifications
- Registered Dietitian or RD eligible
- organizational skills
- excellent communication and interpersonal skills
- experience in LTC and EMR preferred
