Job Summary
Provide nutritional assessments and ongoing evaluation of plans of care for residents in LTC/SNF/subacute rehab facilities; participate in IDCP care conferences; provide nutrition education and counseling to residents, families, and staff; collaborate with interdisciplinary teams to improve residents' health; ensure compliance with federal, state, and local regulations; must be a Registered Dietitian or RD eligible; LTC and EMR experience preferred.
