Job Summary
Perform nutritional assessments and ongoing evaluation of the plan of care for each resident. Participate in IDCP care conferences as needed. Provide nutrition education and counseling to residents, families, and staff. Collaborate with interdisciplinary teams to improve residents' overall health and well-being. Demonstrates ability to work in a cooperative manner as a team member. Completes other duties as assigned. Ensure compliance with federal, state, and local regulations and guidelines.
Required Qualifications
- Registered Dietitian or RD eligible
- organizational skills
- excellent communication and interpersonal skills
- Experience in LTC and EMR preferred
