Job Summary
Conduct nutrition assessments to determine participant needs, preferences, and goals, then actively participate in interdisciplinary team meetings to develop holistic care plans. Deliver and document nutrition interventions, including implementing specific diets and providing counseling to participants and families, while overseeing menu planning and meal delivery programs to ensure regulatory compliance. Manage the nutrition staff through hiring, training, and mentoring, and guide the intake and enrollment process to ensure prospective participants move through the workflow efficiently. This role supports seniors in their homes and communities by making nutrition central to their care journey rather than an afterthought.
Required Qualifications
- Registered Dietitian
- at least two (2) years of experience directing nutrition services in a healthcare setting
- Current Basic Life Support (BLS) and First Aid certification
- Excellent leadership, organization, and communication skills
- At least one (1) year of experience working with a frail or elderly population
