Job Summary
Assess clinical nutrition needs of residents and develop documented care plans within the electronic medical record. Implement individualized care strategies based on lab results, weight trends, and comorbidities while providing nutrition education to patients, families, and staff. Collaborate with interdisciplinary teams to improve overall health and ensure adherence to CMS, State, and Federal regulations. Provide coverage across multiple buildings as needed. Maintain compliance with vaccination policies and hold a valid driver's license if assigned to multi-site locations. Join a supportive team dedicated to enhancing resident experiences at a long-term care facility.
Required Qualifications
- Registered by the Commission on Dietetic Registration.
- Hold a Certification/License in good standing within the state of practice.
- Compliance with COVID-19 vaccination policies as required by state, local, and/or customers.
- If multi-site registered dietitian, must have valid Driver's License
Desired Qualifications
- Knowledge of the state survey process and compliance with nutrition care regulations (preferred, but not required).
