Job Summary
Provide nutrition assessments and develop personalized nutrition care plans for residents; counsel residents and their families on the power of proper nutrition; collaborate closely with nurses, therapists, physicians, and food service teams to ensure holistic care; support special nutritional needs (wound healing, dialysis, dysphagia, malnutrition); ensure regulatory compliance with state, federal, and CMS guidelines; participate in quality improvement initiatives and care planning meetings; maintain accurate documentation and contribute to team discussions; requires a Bachelor’s degree in Nutrition/Dietetics, completion of an accredited Dietetic Internship, and a PA Registered Dietitian license; 2+ years of experience in a long-term care or skilled nursing setting; strong communication and interpersonal skills; familiarity with diverse dietary needs and cultural/religious food practices; tech-savvy with EHRs and data analysis tools; and benefits include a $5,000 sign-on bonus, health plans, PTO, 401(k), and professional development opportunities.
Required Qualifications
- Bachelor’s degree in Nutrition or Dietetics
- completion of an accredited Dietetic Internship
- Current Registered Dietitian license (PA licensure required)
- 2+ years of experience in a long-term care or skilled nursing setting preferred
