Job Summary
Provide nutritional services for in-center and home dialysis patients; perform nutrition assessments, education, and counseling; develop individualized care plans with the interdisciplinary team; monitor labs (albumin, weight, Kt/V) and adjust nutrition therapy; ensure documentation per policies; collaborate on QAI/QEP initiatives; coordinate patient education with case managers; travel between facilities as needed; maintain RD credentials and participate in ongoing education.
Required Qualifications
- Registered Dietitian
- Minimum of 1-year professional work experience in clinical nutrition as a Registered Dietitian
- Current state licensure if applicable
