Job Summary
Provide nutritional services for in-center and home dialysis patients, including education on the relationship between diet and kidney disease and the impact on treatment. Develop and implement individualized nutrition plans with the interdisciplinary team, monitor nutrition-related labs (e.g., albumin, Kt/V), adjust therapy as needed, and document progress in the medical record. Conduct ongoing nutrition assessments, support patient/family/caregiver education, and participate in quality improvement activities and care-plan development. Travel between assigned facilities as required and communicate nutrition concerns with physicians and facility staff to optimize patient outcomes.
Required Qualifications
- Registered Dietitian certification
- Current state licensure if applicable
- Minimum of 1-year professional work experience as a Registered Dietitian
