Job Summary
Provide nutritional services for in-center and home dialysis patients to maximize nutritional status and improve clinical outcomes. Serve as a member of the interdisciplinary healthcare team, delivering patient-specific education on diet, kidney disease, fluid status, and lab parameters; develop and document individualized care plans; monitor nutrition therapy and collaborate on care modifications; travel between assigned facilities as needed; maintain nutrition knowledge and dietetic credentials in accordance with professional standards.
Required Qualifications
- Minimum of 1-year professional work experience in clinical nutrition as a Registered Dietitian
- Current state licensure if applicable
