Job Summary
Provides nutritional services for the facility’s in-center and home patients; educates patients/families about the relationship between diet and kidney disease; participates with the interdisciplinary team to develop an individualized plan of care, assess knowledge of diet and kidney disease, and provide ongoing nutrition assessment and counseling; calculates diet prescriptions, monitors nutrition-related lab parameters (e.g., albumin, weight, Kt/V), and documents care in the medical record; collaborates with care team on management of mineral metabolism, anemia, and sodium/volume control; travels between facilities as required and adheres to licensure regulations and QAI/QEP processes; maintains knowledge through continuing education and complies with certification requirements for a Registered Dietitian; expected to engage in education and communication with patients, families, and other healthcare providers.
