Job Summary
Assess patient knowledge of diet and kidney disease, providing tailored education on the relationship between diet, disease, and treatment. Develop individualized comprehensive plans of care, calculate diet prescriptions, and complete progress notes for chronic dialysis patients per established guidelines. Monitor adherence to nutrition therapy, adjust treatments based on lab results and patient response, and collaborate with the interdisciplinary team on renal bone disease, anemia, and vascular access management. Identify malnourished patients, review monthly albumin and body weight data, and coordinate pre-ESRD education and nutritional supplement referrals. Report on nutrition QAI results and participate in staff meetings to drive process improvements. Requires minimum one year of clinical nutrition experience, previous renal experience preferred, and current state licensure.
Required Qualifications
- Registered Dietitian as per Commission on Dietetic Registration Board Certified Specialist in Renal Nutrition
- Current state licensure if applicable
- Minimum of 1-year professional work experience in clinical nutrition as a Registered Dietitian
- Basic computer skills
- Maintains dietetic registration and continuing education hours as specified by American Dietetic Association and state licensure regulations where applicable
