Job Summary
Conduct initial assessments and follow-up nutrition counseling sessions for patients with chronic diseases like obesity, hypertension, and diabetes. Develop individualized, attainable nutrition plans and provide printed resources to support patient goals. Collaborate with the interdisciplinary care team, primary care providers, and Lifestyle Medicine programs to coordinate referrals for behavioral health, social services, and spiritual care. Lead group educational sessions on specific nutritional topics and promote healthy living at community events. Document all visits in the Electronic Health Record within 24 hours and maintain credentials through continuing education.
Required Qualifications
- Bachelor's degree in Food and Nutrition or Dietetics from an accredited college or university
- The approved practice experience or internship must also have been completed as required by the Academy of Nutrition and Dietetics (AND)
- Registered through the Commission on Dietetic Registration (CDR) at the time of hire or within 6 months of hire
- Receive state licensure of Licensed Dietitian-Nutritionist (LDN) once registration is established
- Bilingual in Spanish/English
- Ability to remain calm under pressure
- Good oral communication skills
