Job Summary
Conduct comprehensive nutritional assessments for residents and patients, then develop, implement, and monitor individualized nutrition care plans. Provide medical nutrition therapy per physician orders while collaborating with physicians, nursing staff, and interdisciplinary teams to ensure optimal outcomes. Monitor and document nutritional status, participate in care plan meetings, and educate residents, families, and staff on nutrition topics. Ensure full compliance with federal, state, and facility regulations. This role supports the network of skilled nursing and rehabilitation facilities by enhancing quality of life through expert nutrition care.
Required Qualifications
- Registered Dietitian (RD/RDN) credential
- Current state licensure (or eligibility)
Desired Qualifications
- Bachelor's degree in Dietetics, Nutrition, or related field
- Experience in long-term care, skilled nursing, or clinical settings
- Strong communication and organizational skills
- Ability to work independently and as part of a team
