Job Summary
Assess the clinical nutrition needs of residents/patients and develop/document individualized care plans within the electronic medical record; provide nutrition education and counseling to residents/patients, families, and staff; collaborate with interdisciplinary teams to determine plans of care to improve health outcomes; ensure adherence to CMS, state, and federal nutrition regulations; provide coverage across multiple buildings based on company needs and as requested; role involves hands-on nutrition care in a long-term care facility with ongoing duties as assigned.
Required Qualifications
- Registered by the Commission on Dietetic Registration
- State license/certification in good standing
- RD-Eligible or provisional licensure accepted in certain locations (as applicable)
- Knowledge of state survey process and nutrition care regulations (preferred)
- Strong organizational and communication skills
- Compliance with COVID-19 vaccination policies as required by state/local/customers
