Job Summary
Conduct clinical assessments, update patient care plans, and complete MDS sections while maintaining knowledge of the Medicare PDPM system. Document in PointClickCare and utilize meal planning software to ensure proper patient dietary needs. Collaborate with chefs, physicians, and the IDT to promote optimal food service operations and diet liberalization. Monitor resident meal intake during rounds, solve meal service issues, and balance nutritional care with cost control. Communicate nutritional recommendations and weight changes to physicians and legal representatives as needed.
Required Qualifications
- Registered Dietitian
- Licensed in the state in which she/he practices
- Detail oriented
- Effective time management skills
- Reliable attendance
- Ability to function independently
- Ability to build rapport with other disciplines
- Flexibility to travel to different facilities from Northern KY to the Dayton area
