Job Summary
Conduct clinical assessments, update patient care plans, and complete MDS sections while maintaining knowledge of the Medicare PDPM system. Document in the electronic medical record and utilize meal planning software to ensure proper patient dietary needs are met. Collaborate with the facility chef, physicians, and the interdisciplinary team to promote optimal food service operations and solve meal service issues. Monitor resident meal intake during rounds, balance nutritional care with cost control, and facilitate diet liberalization for long-term care patients. Communicate nutritional recommendations, weight changes, and new orders to patients, families, and legal representatives as warranted.
Required Qualifications
- Licensed in the state in which she/he practices
- Detail oriented person who ensures processes are completed timely
