Job Summary
Conduct comprehensive nutritional assessments for PACE participants upon enrollment and as needed, utilizing pertinent data to plan and implement appropriate nutrition interventions. Collaborate with the interdisciplinary team to develop and revise care plans, providing nutrition counseling, education, and therapy to participants, caregivers, and family members regarding dietary issues. Monitor weight, medical diagnoses, drug interactions, and hydration status while facilitating interventions such as home-delivered meals and supplements. Participate in interdisciplinary team meetings, quality improvement projects, and family meetings to ensure participant needs are met. Full-time, days, Monday through Friday. Requires a Bachelor's degree in food, nutrition, or dietetics, passing the CDR exam, and at least one year of experience with the elderly population.
Required Qualifications
- Bachelor's degree from an accredited college with major studies in food, nutrition, or dietetics
- Passing the Commission on Dietic Registration (CDR) exam
- Minimum of 1 year working with the elderly population
- Reliable transportation
- Valid driver's license
- Minimum state required liability auto insurance
- Medically cleared for communicable diseases
- All immunizations up to date
- Successful completion of post-offer pre-employment screening and verification processes
- Full Time employment
- Days schedule
- Monday - Friday schedule
Desired Qualifications
- Minimum of two (2) years clinical nutrition experience
- Serve Safe Certified
