Job Summary
Conduct comprehensive nutrition assessments for patients at nutritional risk and develop, implement, and monitor individualized, evidence-based nutrition care plans for those with complex medical conditions. Deliver nutrition education and counseling regarding acute disease management, discharge planning, and readmission prevention while collaborating with physicians, nurses, pharmacists, and other interdisciplinary team members to optimize patient outcomes. Ensure compliance with hospital policies, regulatory standards, and best practice guidelines while maintaining thorough electronic medical record documentation. Monitor patients for acute changes in weight, labs, and fluid status, adjusting interventions promptly, and actively participate in quality improvement initiatives and clinical rounds. Serve as a nutrition expert resource for hospital staff to support education and development.
Required Qualifications
- Bachelor's or Master's degree in Dietetics, Nutrition, or a related field from an accredited program
- Registered by the Commission on Dietetic Registration (RD/RDN)
- Current state licensure/certification (if mandated by the state)
- Minimum of 2-3 years of recent clinical experience in an acute care hospital setting (required)
- Demonstrated experience managing patients with diverse, complex medical conditions
- Proficiency in enteral and parenteral nutrition support
- Exceptional communication and interpersonal skills for effective interaction with patients, families, and high-performing healthcare teams in high-pressure situations
- Possesses superior critical thinking, problem-solving, and decision-making abilities to assess rapidly changing objectives, develop, and implement effective, time-sensitive plans
- Strong familiarity with acute care workflows, electronic health records (EHR), and regulatory standards relevant to hospital accreditation
- Ability to work autonomously and prioritize effectively in a dynamic environment
