Job Summary
Provide nutritional services for in-center and home dialysis patients, educate patients and families on the relationship between diet and kidney disease, and develop individualized nutrition plans. Perform nutritional assessments, monitor lab parameters (e.g., albumin, weight), and adjust plans to achieve clinical goals. Collaborate with the interdisciplinary team to formulate a comprehensive care plan, communicate treatment changes to providers, document and report nutrition QAI results, and participate in quality improvement activities. Maintain required dietetic registration and state licensure where applicable, and travel between facilities as needed. Requires Registered Dietitian credentials and ongoing professional development.
Required Qualifications
- Registered Dietitian credential (RD)
- Current state licensure (where applicable)
- Minimum 1 year professional experience in clinical nutrition
- Ability to provide education on nutrition and kidney disease and to collaborate with interdisciplinary team
