Job Summary
Assess and evaluate the nutritional needs of residents in a Skilled Nursing Facility, then create, implement, and monitor individualized nutrition care plans. Provide counseling and education to residents, families, and staff while partnering with nursing, therapy, and dietary staff to ensure coordinated care. Document assessments, progress, and updates in medical records and ensure compliance with federal, state, and facility dietary regulations. Support food service operations to maintain safety, quality, and satisfaction. Required Qualifications
- Licensed Registered Dietitian (RD) in good standing
- Bachelor's degree in Nutrition, Dietetics, or related field

- Resident-centered approach to care
- Compliance with federal, state, and SNF dietary regulations
Desired Qualifications
