Job Summary
Coordinate nutritional care for residents by completing nutritional observations/reviews, developing and implementing care plans, and documenting dietary information under the supervision of the Registered/Licensed Dietitian. Responsibilities include completing nutritional observations/reviews, MDS, CAAs and care plans; updating progress notes; visiting residents to assess intervention effectiveness and preferences; reviewing weight and dietary needs; promoting liberalized diets; ensuring timely follow-up on physician recommendations; communicating nutrition changes during 24-hour clinical handoffs; participating in interdisciplinary team meetings; and serving as Dining Room Monitor as required.
