Regisstered Nurse - Hospice of Cincinnati Support Team

Conduct telephone triage, clinical assessment, and care coordination for hospice patients and families during overnight hours. Respond to after-hours patient and caregiver calls, assess symptoms, provide interventions and education, and coordinate visits. Complete SIA visits for patients in the final seven days of life and perform direct, indirect holistic care. Collaborate with physicians and the healthcare team to revise care plans, delegate tasks, and ensure timely access to support. Drive to patient locations for field-based care while maintaining patient autonomy and documenting all interventions per guidelines.
