Job Summary
Coordinate regional guidance and intake processes in the ZOU area, assessing new referrals and conducting trajectory conversations to ensure clients reach appropriate support. Maintain oversight of inflow, waiting lists, and outflow while collaborating with individual guides, municipalities, and housing corporations to solve complex cases. Represent the Tussenvoorziening in regional meetings to align on changing care needs, including ambulant living and decentralized housing models. Work directly with vulnerable populations facing homelessness or mental health challenges, utilizing knowledge of Wmo and Wlz legislation.
Required Qualifications
- Een hbo-diploma (zoals Social Work, MWD, SPH of Toegepaste Psychologie)
- ervaring met sociaal kwetsbare doelgroepen, zoals mensen die te maken hebben met dak- en thuisloosheid, OGGZ-problematiek, psychiatrie of verslaving
- Kennis van de wetgeving binnen het sociaal domein (Wmo en Wlz)
- Kennis van methodieken binnen de sector, bij voorkeur Krachtwerk
- Een rijbewijs B
- de mogelijkheid om met eigen vervoer de regio te bezoeken
Desired Qualifications
