Job Summary
Respond to service requests and obtain physician orders for patient evaluations. Supervise and train personnel such as PTAs or aides while establishing written plans of care with measurable goals. Administer and supervise the LPTA, maintain accurate daily and weekly progress notes, and modify treatment plans as indicated. Prepare work settings, equipment, and supplies, and communicate with referring physicians and rehab team members regarding total treatment programs. Maintain a productivity rate of at least 75% and submit all employee paperwork to corporate timely. Willing to travel to assigned locations, which may include overnight stays.
Required Qualifications
- Must be a graduate of an accredited Physical Therapy program
- Must have a high level of cognitive and perceptual skills
- Must have a valid Driver's License in the state of employment
- Willing to travel to assigned locations, which may include overnight stays
Desired Qualifications
- SNF experience preferred
