Job Summary
Respond to service requests, obtain physician orders, and evaluate patients to establish written plans of care with measurable goals. Supervise and train personnel such as PTA or aides, while administering and supervising the LPTA post-evaluation. Maintain accurate daily and weekly progress notes, ensuring all documentation is complete, accurate, and timely. Assess treatments, modify the plan of care as indicated, and prepare work settings with equipment and supplies. Communicate with referring physicians and rehab team members regarding the total treatment program. Maintain a productivity rate of at least 75% and follow all company policies and procedures. Willing to travel to assigned locations, which may include overnight stays.
Required Qualifications
- Must be a graduate of an accredited Physical Therapy program
- Must have a high level of cognitive and perceptual skills
- Must have a valid Driver's License in the state of employment
- Willing to travel to assigned locations, which may include overnight stays
Desired Qualifications
- SNF experience preferred
