Job Summary
Oversee psychosocial services and therapeutic activity programs across multiple long-term care facilities, ensuring regulatory compliance with federal regulations including F-Tags and CMS standards. Monitor and audit social service documentation, assessments, and care plans while supporting completion of psychosocial assessments and discharge planning processes. Provide leadership, training, and operational support to facility Social Services and Activities Directors, conducting competency evaluations and developing best-practice toolkits. Participate in survey preparation for state and federal reviews, track quality measures, and develop corrective action plans to improve resident satisfaction scores. Travel regularly to assigned facilities to assist with staffing assessments and interim support during vacancies.
Required Qualifications
- Bachelor's degree in Social Work, Human Services, Therapeutic Recreation, or related field
- Minimum 3–5 years' experience in long-term care
- Minimum 2 years in a leadership role (Social Services Director or Activities Director)
- Strong knowledge of CMS regulations for long-term care
- Experience with MDS 3.0 documentation related to psychosocial and activity sections
- Ability to travel frequently within assigned region
- Must be able to continuously deal effectively with stress created by residents, multiple tasks, noises, interruptions, and work cooperatively as part of the health care team while maintaining a pleasant attitude
- Must be able to lift (minimum weight of 5 lbs. and maximum weight of 100 lbs.)
- Must be able to lift (typical weight of 5 lbs. and maximum weight of 30 lbs.)
