Job Summary
Conduct interviews with applicants to gather information for SSI/SSDI applications and prepare Medical Summary Reports according to Program guidelines. Accompany participants to virtual and in-person Social Security Administration appointments while coordinating with medical specialists to obtain necessary evidence. Determine participant eligibility for SOAR services, carry out utilization review and quality assurance activities, and maintain documentation for program evaluation. Provide housing case management as needed and meet billing productivity requirements while attending team meetings and cross-systems training. Serve clients across Ross, Pickaway, Clinton, Highland, and Fayette Counties with routine travel common but overnight travel not required. Obtain SOAR Ohio Certification within six months and maintain it annually.
Required Qualifications
- Bachelor's degree in a related field, certification as a peer specialist, or minimum three years of directly relevant experience
- Cursory knowledge of medical and psychiatric terminology and ability to communicate using same
- Experience with individuals experiencing homelessness
- Demonstrated high degree of cultural awareness
- Licensed and able to operate a motor vehicle with appropriate level of insurance coverage
- Able to effectively communicate through verbal/written expression
- Must be able to operate in an Internet-based, automated office environment
- Must obtain SOAR Ohio Certification within 6 months
- Must maintain SOAR Certification each year, and attend all training as required
- Must be able to lift 50 lbs
