Job Summary

Provides patient care services within a regional group of Liberty Home Health & Hospice offices and documents patient care data. Performs initial psycho-social assessment to develop an individualized care plan and establishes follow-up treatment goals. Applies psycho-social principles and family dynamics to care planning; identifies and reassesses social, emotional, financial, and environmental factors affecting the medical plan. Communicates with intercompany contacts; utilizes community resources and assists with discharge instructions. Travels as required to support regional offices. PRN role requiring a Master’s degree in social work and eligibility for NC social work certification; at least one year of healthcare social work experience and willingness to travel. Other duties as assigned.

Required Qualifications

  • Master’s degree from a school of social work that is accredited by the Council on Social Work Education and eligible for certification by the North Carolina Board of Social Work
  • Baccalaureate degree in social work, psychology, sociology, or other field related to social work, and has at least one (1) year of social work experience in a healthcare setting
  • Minimum of one year practical experience, preferably in the home health care environment
  • Strong interpersonal skills, with the ability to effectively communicate within a multidisciplinary setting
  • Must accept travel and working conditions as assigned

Desired Qualifications

  • Master’s degree from a CSWE-accredited school of social work and eligible for certification by the North Carolina Board of Social Work
  • Bachelor’s degree in social work, psychology, sociology, or related field with at least one year of social work experience in a healthcare setting
  • Minimum of one year practical experience, preferably in the home health care environment
  • Strong interpersonal skills and ability to communicate within a multidisciplinary setting
  • Willingness to travel and adapt to assigned working conditions
