Regional Director of Facility Partnerships and Clinical Support

Job Summary
Conduct initial on-site meetings with facility administrators, nursing leadership, and clinical teams to assess wound care structures, staffing models, and operational needs. Establish trusted relationships with Skilled Nursing, Assisted Living, and Independent Living partners by presenting the clinical model, defining referral expectations, and aligning services with specific resident-care priorities. Execute territory-specific growth strategies through prospecting, educational sessions, and partnership development to expand patient census and referral activity. Lead facility onboarding by developing implementation plans, educating staff on workflows and eligibility criteria, and coordinating seamless transitions with internal clinical and operational teams. Monitor active patient census, treatment volume, and utilization metrics to identify gaps, address implementation barriers, and drive performance improvements. Serve as the primary escalation point for scheduling, documentation, and service-delivery issues while maintaining compliance with HIPAA and healthcare regulations.
