Job Summary
Lead the region's health services by managing, supervising, and developing client care teams to ensure quality delivery within state and federal laws. Independently direct Nurse Case Managers, oversee fiscal affairs, and coordinate performance improvement activities while reviewing data to ensure accuracy and consistency. Assign clinical personnel to clients, attend case conferences, and facilitate problem-solving sessions for reimbursement and care coordination issues. Ensure 24-hour coverage, maintain current local contracts, and enforce human resource policies including hiring recommendations and performance evaluations. Collaborate with administration on budgetary needs, assist in marketing and discharge planning, and designate written alternates for absences.
Required Qualifications
- Graduate of a state-approved school of professional nursing
- Current unblemished and unrestricted license as a Registered Nurse in the state(s) of practice
- Minimum of five (5) years nursing experience including home care or closely related service
- Minimum of three (3) years of management experience
- One to two (1-2) years case management experience
- Must be a licensed driver with automobile insured in accordance with Company and state requirements
- Current CPR certification
- Ability to travel
- Knowledge of home care federal and state regulations
- Knowledge of reimbursement sources and documentation requirements within home health care
Desired Qualifications
- BSN or MSN preferred
- Excellent communication (verbal and written), interpersonal and leadership skills, and strong business acumen
- Demonstrates organizational, facilitative and assertiveness skills, flexibility and the ability to work effectively in teams
- Strong analytical skills and ability to summarize and present data and statistics
