Job Summary
Coordinate all referrals and follow-up care for PACE participants by receiving, processing, and scheduling medical requests for diagnostic testing, specialists, and other providers. Arrange transportation and escort coordination for participants to medical appointments, while tracking referrals in electronic medical records and maintaining consistent status updates on authorization requests. Complete surgery scheduling with proper CPT codes, manage pre and post orders, and submit retro-authorizations to ensure timeliness and avoid care lapses. Screen referral calls via telephone, text, and patient portal, acting as a liaison between participants, providers, and community resources to advocate for the referral process.
Required Qualifications
- High school diploma/GED
- One year of clinical or healthcare experience
- Current Basic Life Support (BLS) certification
- Current CPR and First Aid certification
Desired Qualifications
- Experience with referral authorization and data processing
- One year of experience working with elderly populations
- Bilingual (English/Spanish)
- Excellent verbal and written communication skills
- Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
- Knowledgeable about and experience with current procedural terms (CPT), international classification of diseases (ICD-10), and medical terminology
- Ability to successfully manage multiple tasks simultaneously
- Excellent planning and organizational ability
- Ability to work as part of a team as well as independently
- Ability to work with highly confidential information in a professional and ethical manner
