Job Summary
Hospice Liaison/Admissions Nurse coordinates hospice admissions from hospitals, healthcare facilities, physicians, and community referral sources. Serves as clinical consultant and educator to referral sources; educates patients and families about hospice services and levels of care; develops and coordinates initial care plans for patients admitted to Home Hospice or General Inpatient Hospice; obtains admission orders and coordinates certification of terminal illness with Hospice Medical Directors; ensures timely, compliant admissions and maintains knowledge of regulatory and reimbursement guidelines; builds relationships with referral sources and partners, and participates in quality improvement initiatives.
Required Qualifications
- RN license or current licensure as a registered nurse
- Hospice admissions experience (preferred)
- Knowledge of hospice regulations, reimbursement guidelines, and regulatory requirements
- Strong communication and collaboration skills with patients, families, physicians, discharge planners, and referral sources
