Job Summary
Schedule and confirm patient consultation and referral appointments with specialists by evaluating financial, accessibility, and medical needs to determine appropriate specialty care. Accurately document all referral steps within the Electronic Health Record, including appointment scheduling, notification attempts, and record collection efforts. Track referrals to ensure documentation is obtained for continuity of care, attaching specialty records to referrals and sending them to referring providers. Organize and aggregate incoming patient health information to create historical orders and support structured record completion. Communicate personal health information effectively with external organizations to support high-quality patient care while maintaining HIPAA compliance.
Required Qualifications
- High school diploma or GED
- Must be organized and able to manage/prioritize tasks to meet deadlines
- Must be able to adapt to change in a positive manner
- Must be able to work as a part of a team
- Must be computer literate, especially with Microsoft Office products
- Must be proficient with the Electronic Health Record software
- Must be able to maintain good inter-personal relationship with co-workers and other members of the health care team
- Must be able to maintain patient confidentiality in accordance to organization's policy and procedure and HIPAA requirements
- Must regularly lift and/or move up to 10 pounds
- Must have close vision
- Must have peripheral vision
Desired Qualifications
- At least 1 year of experience in healthcare setting preferred
- Familiarity with healthcare – especially medical - terminology
- Social and cultural sensitivity appropriate to ethnically and economically diverse patient-and employee-base
- Proficient in Microsoft
