Referral Services Assistant

Initiate and manage clinical referrals for participants in disease management programs, acting as a liaison between hospitals, health plans, physicians, and vendors. Review submissions for completeness, verify insurance coverage, obtain authorizations, and assign cases to staff. Contact physician offices for demographic data and enter all referrals and communications into the system. Handle multiple tasks including calls, texts, faxes, and electronic queues while maintaining detailed notes. Requires a high school diploma, one year of healthcare experience, and proficiency with digital tools. Full-time virtual work with occasional in-person training and onsite requirements.
