Job Summary
Referral Coordinator supports care delivery teams by coordinating patient referrals to other healthcare providers and organizations, ensuring timely access to referred resources, facilitating appointment coordination, obtaining necessary reports, and performing post-appointment follow-up. Responsibilities include processing referrals within priority-based timeframes, obtaining health plan pre-authorizations as needed, providing required documentation to referred entities, updating and tracking referrals in the EHR, coordinating appointment scheduling with patient advocacy to ensure timely attendance, delivering high-value customer service for patient inquiries, ensuring timely receipt of specialist/post-appointment reports, and contributing to Lean and quality-improvement activities. Required qualifications emphasize medical terminology, health-care experience (minimum two years), strong communication and organizational skills, familiarity with regulatory requirements, proficiency with MS Office and project-management tools, and experience with EHR systems (eClinicalWorks or equivalent). This role is full-time and non-exempt, located in Chula Vista, CA.
Required Qualifications
- Medical Certification from an accredited program preferred
- Proficiency in medical terminology and clinical processes
- At least two years' experience in the health care field
- Ability to manage multiple, complex requests with strict deadlines
- Demonstrated analytical ability in identifying problems, developing solutions and implementing effective courses of action
- Strong communication skills—verbal and written
- Demonstrated ability to work effectively with varied levels of personnel and with professionals and business organizations
- Extensive knowledge of regulatory requirements
- Strong knowledge of MS Office and Project Management tools
- Excellent organizational and interpersonal skills with professional work ethic
- Experience with eClinicalWorks (or equivalent EHR)
