Job Summary

As a Referral Coordinator, you will handle the intake process for healthcare services, liaising between the company, patients, and health plans. Responsibilities include verifying patient information, insurance verification, authorization/referral processing, obtaining and releasing clinical documentation, responding to health plan inquiries, and resolving patient accounts. Success in this role requires 2+ years of intake referrals experience within Home Health, effective communication skills, proficiency in EMR/EHR applications, self-motivation, and strong time management abilities.

Required Qualifications

  • 2+ years of intake referrals experience within Home Health
  • Effective verbal and written communication skill
  • Ability to use various computer programs and applications, IE EMR/EHR
  • Ability to self-motivate and work independently
  • Ability to multi-task, set priorities and manage time effectively

Desired Qualifications

  • Bilingual Spanish a plus
  • Certification a plus
