Job Summary
The Reentry Targeted Case Manager (rTCM) coordinates person-centered, recovery-focused care for justice-involved Apple Health (Medicaid) clients during the pre-release period. Responsibilities include conducting comprehensive reentry assessments across medical, behavioral health, substance use, and social support needs; developing individualized care plans with referrals, medications, follow-ups, and community supports; maintaining clinical documentation for Medicaid billing; coordinating warm hand-offs to primary care, behavioral health, MAT/SUD programs, and social services; assisting clients with Medicaid enrollment and release logistics; educating clients on medication adherence, chronic disease management, mental health follow-up, and community resources; and collaborating with internal teams and external providers to ensure continuity of care and regulatory compliance. The role requires active RN licensure, adherence to HIPAA and state/facility requirements, and participation in quality improvement and reporting efforts. Required Qualifications
- Active RN licensure
- Bachelor’s degree in a relevant field preferred (HR, Finance, Sales, Business Administration, Healthcare Administration)
- Minimum 2-3 years of relevant experience in functional area (HR, Finance, Sales, Marketing, Operations)
- Experience in healthcare/correctional healthcare or regulated industry preferred
- Knowledge of regulations, compliance standards in regulated functions (HR/Finance) and related certifications (PHR, SHRM-CP, CPA) may be required
- Active CPR/BLS certificate required if role is located in a correctional facility
