Job Summary
Recovery Support Navigator provides clinical case management to engage and support patients seeking treatment upon release and their families, to motivate and maintain them in culturally relevant behavioral health services in the community. Engagement begins 120 days pre-release and continues up to 90 days post-release. Activities include coordinating transportation and housing resources, making referrals as needed, working on-site or in the community, and communicating with referral sources and treatment entities using releases of information. The role requires independent work and teamwork, and occasional travel to other programs or locations to meet business needs.
Required Qualifications
- High school diploma required
- LADC-II preferred
- Bachelor’s degree in human services or related field or equivalent experience preferred
- Case management experience strongly preferred
