Job Summary
Recovery Support Navigator in the MOUD program within the Department of Corrections. Responsibilities include providing clinical case management to engage, educate and support patients seeking treatment upon release and their families to motivate and maintain them in culturally relevant behavioral health services in the community. Engagement begins 120 days pre-release and continues up to 90 days post-release. Works independently and as part of a team, assists patients with transportation and housing resources, coordinates treatment with referrals using releases of information, and may travel to other programs as needed. On-site work with potential community outreach; flexible hours to meet organizational needs.
Required Qualifications
- High school diploma required
- LADC-II preferred
- Bachelor’s degree in human services or related field or equivalent experience in the behavioral health field preferred
- Case management experience strongly preferred
