Job Summary
Collaborate with a multidisciplinary care team to coordinate recovery support services for a panel of 160-180 members, driving treatment engagement and seamless care across clinical, social, and community settings. Build trusted relationships to assist members in achieving recovery goals through motivational interviewing, while supporting unengaged members with intensive outreach and new members through transition efforts. Link individuals to healthcare, behavioral health, and community resources, helping them navigate practical systems like insurance, pharmacy, and housing. Maintain accurate EHR documentation, manage referrals and releases of information, and facilitate smooth care transitions for special populations. Provide in-person, virtual, and telephonic support while participating in weekly interdisciplinary team meetings and continuing education. Work Monday through Thursday, 9am-7pm or 10am-8pm, with Wednesday hours completed from home.
Required Qualifications
- High school diploma
- Reliable internet access (50M download / 10M upload) and strong WiFi if working remote
- Must meet state-specific background screening and pre-employment requirements (which may include fingerprinting, drug testing, CPR/First Aid, or health screening)
- Valid driver's license and access to a vehicle (if role includes community travel)
Desired Qualifications
- Associate's degree in Social Services, Human Services, Psychology, or related field
- Bachelor's degree in related field (Virginia and Tennessee)
- Associate's degree with one year experience; or MHRT/C or Community Health Worker certification (Maine)
- Peer Support Specialist / Peer Recovery Coach certification
- Minimum of two (2) years' experience in case management, care coordination, peer support, or a related field
- Experience working with individuals with substance use or mental health challenges
- Experience using Electronic Health Records (EHR)
- Commitment to Groups' mission and values, including member-centered care and recovery support
- Ability to work evenings or flexible hours as needed
