Recovery Support Coordinator

Job Summary

Collaborate with a multidisciplinary care team to coordinate recovery support services for a panel of 160-180 members, driving treatment engagement and seamless care across clinical, social, and community settings. Build trusted relationships using motivational interviewing to assist members in achieving recovery goals, while providing intensive outreach to reengage unengaged individuals and supporting new members through transition to care. Deliver in-person, virtual, and telephonic assistance by linking members to healthcare, behavioral health, and social resources, and help navigate practical systems like insurance, pharmacy, and housing. Maintain accurate EHR documentation, manage referrals and releases of information, and participate in weekly interdisciplinary team meetings to ensure coordinated service delivery and removal of barriers to care.

Required Qualifications

  • High school diploma
  • Associate's degree in Social Services, Human Services, Psychology, or related field
  • Peer Support Specialist / Peer Recovery Coach certification
  • Minimum of two (2) years' experience in case management, care coordination, peer support, or a related field
  • Experience working with individuals with substance use or mental health challenges
  • Experience using Electronic Health Records (EHR)
  • Reliable internet access (50M download / 10M upload) and strong WiFi
  • Valid driver's license and access to a vehicle
  • Must meet state-specific background screening and pre-employment requirements
  • Florida Background Screening Requirement: As a condition of employment for positions providing care to vulnerable populations, the selected candidate must undergo a background screening through the Florida Care Provider Background Screening Clearinghouse

Desired Qualifications

  • Associate's degree in Social Services, Human Services, Psychology, or related field preferred
  • Peer Support Specialist / Peer Recovery Coach certification may be preferred or required in select programs or states, if applicable
  • Minimum of two (2) years' experience in case management, care coordination, peer support, or a related field preferred
  • Experience working with individuals with substance use or mental health challenges preferred
