Job Summary
Coordinate recovery support services for participants by assisting with access to shelters, medical services, food, and substance use treatment. Complete needs assessments, triage tools, and document all contacts using electronic health records. Facilitate connections to the recovery community, peer groups, and life skills training while driving patients to appointments. Maintain professional boundaries, intervene in high-risk situations, and promote community integration across housing, employment, and social support areas. Provide short-term relief coverage and participate in team meetings and trainings. This on-call position covers shifts in sobering to reinforce treatment goals and access resources.
Required Qualifications
- High school diploma or GED
- CRM certification at time of hire
- Basic Life Support (BLS) certification or ability to acquire within 90 days of hire
- Crisis Prevention training completion within 90 days of hire
- A valid Oregon or Washington driver's license
- Ability to safely operate a motor vehicle, including a passenger van for residential settings
- Ability to perform Basic Life Support for 15 minutes continuously if certified in BLS
- Ability to lift up to 25 lbs
- Ability to follow infection control practices and safely work in patient-focused environment
- Ability to negotiate a staircase, reach, and handle objects
- Ability to sit for extended periods of time
- Ability to operate a personal computer for extended periods of time
- Ability to work independently and within a multi-disciplinary team
- A negative result on all pre-employment, reasonable basis, and post-accident drug screens
- An acceptable criminal record as determined by DHS, the Oregon Administrative Rules, and the Oregon Revised Statutes; and no current supervised parole or probation
- An acceptable driving record and automobile insurance coverage as recommended by CODA's automobile insurance provider
